GynecomastiaThe presence of excessive breast tissue in a man is called "gynecomastia". Although it is a very common problem, for many men it is deeply embarrassing. It can cause intense emotional pain and may prevent you from participating in activities such as swimming and other sports. You may even feel uncomfortable taking your shirt off in view of your spouse, or you may wrap your chest or assume an posture to limit the visibility of the breast tissue. If you have gynecomastia, know that you are not alone, that this is actually quite normal, and it can be corrected.

Causes of gynecomastia

Men can develop breast tissue for many reasons and at different times in life. Gynecomastia often occurs during puberty and resolves by itself, so I do recommend that patients under age 18 have a work-up by their pediatrician prior to considering plastic surgery. Excess breast tissue created by weight gain will also usually go away on its own with weight loss, but some extra tissue or skin may remain.

Although rare, drug use or a disease process is sometimes the underlying cause of gynecomastia. Typically, this will cause asymmetrical breast growth. It is important to rule out any underlying health problems before breast reduction surgery. Depending on the cause, surgery may or may not be appropriate or necessary.

Gynecomastia can be caused by:

  • Hormonal changes such as puberty
  • Steroids and certain medications
  • Marijuana use
  • Other hormonal imbalances
  • Obesity
  • Aging
  • Thyroid disorders
  • Genetic disorders
  • Tumors
  • Cancer
  • Chronic liver disease
  • Klinefelter Syndrome
  • Gilbert's Syndrome
  • Castration

Gynecomastia treatment:

The method of reducing your breasts will depend on the nature of their growth and the type of tissue involved. Excess fat can be removed with liposuction. Excess glandular tissue must be surgically removed. This tissue is often firm and right behind the nipple, necessitating only a small incision. In some cases, larger incisions will be required, but I always try to limit this as these incisions can be quite visible on the male chest. Most men have both soft fat and denser breast tissue, and therefore require a combination procedure.
